Payment and Insurance
Payment is due at the time the service is provided. Please pay using cash, check, credit card, or debit card. We accept Visa, MasterCard, Discover, and American Express. We also accept Care Credit.
As a courtesy, we will bill your insurance carrier if you provide us with the proper paperwork. Co-payments and deductibles are due at the time of service. Your agreement with the insurance carrier is between you and your insurance company, and as such we do not routinely research denied claims or claims not paid per contracted agreement. If your insurance carrier has not paid within 60 days from the date of billing, you will be responsible for the outstanding balance. Any services provided which are not covered under your insurance plan must be paid in full at the time of your visit or upon notice of insurance claim denial. Some procedures we perform may be covered only by your medical insurance and some are covered only by your dental insurance. On occasion, both your medical and dental insurance may offer coverage.
Any account not paid according to our request will be turned over to a collection agency. This amount must be paid in full before any further appointments can be made.
